Are you evaluating a patient for heart failure in a prehospital setting?

Does the patient have a history of?

• rest dyspnea?

• orthopnea?

• paroxysmal nocturnal dyspnea?

• dyspnea on walking on level?

• dyspnea on climbing?

Findings on physical examination:

Heart rate

beats per minute

Jugular venous pressure

cm water

Is hepatomegaly present?

Is edema present?

Are basilar lung crackles present?

Are lung crackles present above the basilar segments?

Is wheezing present?

Is a third heart sound present?

Does the patient?

• have the hepatojugular reflex?

• have ECG changes (old AMI, nonspecific ST-T changes, arrhythmia, left ventricular hypertrophy)?

• cough at night?

• have a cardiac murmur?

• have sputum production or fever?

• have a history of previous AMI, arrhythmia or CHF?

• take medications for heart failure?
