Description

Senni et al developed an index for predicting the one year mortality rate for a patient with congestive heart failure. This is based on the severity of the heart failure and the presence of comorbidities. The authors are from Ospedali Riuniti, Ospedale Niguarda, Cambridge University, Universita di Pavia and Societa Italiana di Medicina Generale in Italy.

Parameters:

(1) age in years

(2) cancer

(3) complicated diabetes mellitus

(4) beta blocker therapy

(5) angiotensin-converting enzyme inhibitor or angiotensin receptor blocker

(7) left ventricular ejection fraction

(8) valvular heart failure

(9) anemia

(10) chronic obstructive lung disease (COPD)

(11) renal dysfunction

(12) atrial fibrillation

(13) hypertension

where:

• Anemia = hemoglobin concentration < 11 g/dL (not adjusted for gender).

• Moderate to severe renal dysfunction = serum creatinine > 2 mg/dL, dialysis, post-renal transplant, or uremia.

• Complicated diabetes mellitus = retinopathy, neuropathy, or nephropathy.

• Hypertension = systolic blood pressure >= 140 mm Hg or diastolic pressure >= 90 mm Hg

total index =

= SUM(points for all 13 parameters)

Interpretation:

• minimum score: -1

• maximum score: 25 or more

• The higher the score the higher the one year mortality rate.
