Parameters:

(1) age in years

(2) cancer

(3) complicated diabetes mellitus

(4) beta blocker therapy

(5) angiotensin-converting enzyme inhibitor or angiotensin receptor blocker

(6) New York Heart Association (NYHA) class

(7) left ventricular ejection fraction

(8) valvular heart failure

(9) anemia

(10) chronic obstructive lung disease (COPD)

(11) renal dysfunction

(12) atrial fibrillation

(13) hypertension

where:

• Anemia = hemoglobin concentration < 11 g/dL (not adjusted for gender).

• Moderate to severe renal dysfunction = serum creatinine > 2 mg/dL, dialysis, post-renal transplant, or uremia.

• Complicated diabetes mellitus = retinopathy, neuropathy, or nephropathy.

• Hypertension = systolic blood pressure >= 140 mm Hg or diastolic pressure >= 90 mm Hg

Interpretation:

• minimum score: -1

• maximum score: 25 or more

• The higher the score the higher the one year mortality rate.

Performance:

• The area under the ROC curve was 0.81 to 0.84 in the validation and derivation groups respectively.
