Mocroft et al developed a staging system for patients with AIDS based on the number and type of AIDS diseases and the CD4 lymphocyte count. This can be tracked over time to monitor disease progression and response to therapy.
Diagnostic groups:
(1) mild diagnoses
(2) severe diagnoses
(3) very severe diagnoses
Mild diagnoses:
(1) esophageal candidiasis
(2) cutaneous Kaposi's sarcoma
(3) Pneumocystis carinii pneumonia
(4) extrapulmonary tuberculosis
Severe diagnoses (AIDS defining disorders not included in the mild and very severe groups):
(1) candida of bronchus, trachea or lungs
(2) invasive cervical carcinoma
(3) disseminated or extrapulmonary coccidioidomycosis
(4) disseminated or extrapulmonary histoplasmosis
(5) extrapulmonary cryptococcosis
(6) chronic cryptosporidiosis (> 1 month duration)
(7) chronic isosporiasis (> 1 month duration)
(8) HIV-related encephalitis
(9) cytomegalovirus infection other than liver, spleen or lymph nodes
(10) CMV retinitis
(11) Herpes simplex with chronic ulcers (> 1 month duration), bronchitis, pneumonitis, or esophagitis
(12) disseminated or extrapulmonary atypical mycobacteria infection
(13) recurrent pneumonia (2 or more episodes within a 1 year period)
(14) progressive multifocal leukoencephalopathy
(15) recurrent Salmonella septicemia
(16) toxoplasmosis of brain
(17) wasting syndrome due to HIV
Very severe diagnoses:
(1) malignant lymphoma
score =
= (300 * (number of very severe diagnoses)) + (100 * (number of severe diagnoses)) + (20 * (number of mild diagnoses)) - (minimum CD4 lymphocyte count per µL since onset of AIDS)
