Evaluation

Has the patient been diagnosed as having sepsis?

Does the patient have a wound infection with purulent discharge or entero-cutaneous fistula requiring only light dressing changed once daily?

Does the patient have a wound requiring to be dressed with a pack?

Does the patient have a wound that needs to be changed more than once a day?

Does the patient have a wound requiring application of a bag and/or requiring suction?

Does the patient have peritonitis?

Is the peritonitis generalized?

Does the pateint have clinical or radiological signs of chest infection?

Does the patient have full manifestations of lobar or bronchopneumonia?

Does the patient have cough with production of a purulent sputum?

Does the patient have a deep seated infection such as subphrenic abscess, pelvic abscess, empyema thoracis, acute or chronic osteomyelitis?

Maximum daily temperature

Minimum daily temperature

Number of temperature peaks > 38.4° C in 1 day

Maximum number of rigors in one day

Does the patient have obvious jaundice in the absence of established hepatobiliary disease?

Does the patient have a metabolic acidosis?

Is the acidosis uncompensated?

Is the patient in renal failure?

Does the patient have a gross disturbance of mental orientation, decrease in the level of consciousness or a focal neurologic deficit ascribable to the infection and not to some other cause?

Does the patient have a bleeding diathesis due to DIC?

Number of positive blood cultures

Were positive cultures collected that were more than 24 hours apart?

Does the patient have a history of a recent invasive procedure?

Does the patient have a cardiac murmur?

Does the patient have a tender, enlarged spleen?

White blood cell count

Hemoglobin level (in the absence of obvious bleeding)
