Description

McCarthy et al used an activities assessment scale to evaluate impairment secondary to a hernia. This can be used to evaluate the patient before and after surgery. The authors are from Northwestern University, University of Illinois, Creighton University, University of Utah and the Veterans Administration.

NOTE: The scale was applied to "hernia" and did not specify inguinal, umbilical or incisional.

Difficulty in the following activities during the last 24 hours:

(1) lying in bed

(2) sitting

(3) getting in or out of a bed or chair

(4) reaching or stretching

(5) lifting an object weighing 3 to 5 pounds (1.4 to 2.3 kg)

(6) walking around inside

(7) climbing up or descending stairs

(8) walking outside or at work

(9) engaging in sedentary activities

(10) engaging in light physical activities

(11) engaging in moderate physical activities

(12) engaging in vigorous physical activities

(13) engaging sexual intercourse

where:

• For the implementation you could add an activity for lifting a heavy object.

• For lifting I added "from ground to a table.".

• Sedentary activities include typing, talking, playing cards, watching TV.

• Light physical activities include cooking, dusting, clerical work or visiting friends.

• Moderate physical activities include sweeping, washing the car, dancing, hiking, or playing golf.

• Vigorous physical activities include construction work, shoveling, weight lifting, playing vigorous sports.

where:

• The original scale is scored from 1 to 5, but since the final calculations require an adjustment down the above point assignment is cleaner.

total score =

= SUM(points for all applicable items)

Interpretation:

• minimum score: 0

• maximum score (if all 13 activities): 52

• The assessment can be performed before and after hernia to surgery to determine functional improvement.

functional impairment in percent =

= (total score) / (4 * (number of applicable items)) * 100%
