Description

Martini et al identified prognostic factors affecting the survival of a patient with juvenile systemic sclerosis. This can help to identify a patient who may benefit from more aggressive management. The authors are from the Juvenile Scleroderma Working Group of Paediatric Rheumatology European Society (PRES).

Factors associated with a poor prognosis:

(1) diffuse form of systemic sclerosis with rapid progression

(2) pulmonary fibrosis seen on lung imaging studies

(3) elevated serum creatinine

(4) pericarditis

The interval between onset of symptoms and diagnosis has conflicting significance. On the one hand a short interval is said to be protective. On the other hand it is stated that a short interval indicates rapidly progressive disease with a short survival.
