• an active malignancy?

• current or recent cytotoxic chemotherapy?

• current or recent immunosuppressive therapy?

• current or recent therapy with a targeted therapy (TNF-alpha inhibitor, other)?

• current or recent therapy with disease-modifying drugs (methotrexate, azathiorprine, cyclosporine)?

• current or recent therapy with systemic corticosteroids for more than 30 days?

• HIV?
