Description

Fagotti et al developed a score for identifying women with advanced ovarian cancer who will have suboptimal debulking of intraperitoneal metastases. These can help to identify a patient who should undergo surgery. The authors are from Catholic University of the Sacred Heart in Campobasso, Italy.

Patient selection: woman with advanced ovarian cancer

Parameters:

(1) omental cake

(2) peritoneal carcinosis

(3) diaphragmatic carcinosis

(4) meseneteric retraction

(5) bowel infiltration

(6) stomach infiltration

(7) liver metastases

Status for Each Parameter

Points

absent

0

present

2

predictive index =

= SUM(points for all 7 parameters)

Interpretation:

• minimum predictive index: 0

• maximum predictive index: 14

• The higher the score the less likely that the patient will be optimally debulked at surgery.

• The authors selected a predictive index >= 8 for identifying a patient who cannot be optimally debulked.

Performance:

• The performance data is given in Table 2 on page 1159.

• The highest Youden score and diagnostic accuracy is for a score >= 4 with a sensitivity of 61% and specificity of 87%.

• A predictive index >= 8 showed 100% specificity (no one "inappropriately unexplored").
