• on oral contraceptive or hormone replacement therapy?

• been pregnant or had a delivery in past month?

• have a history of unexpected stillbirth, recurrent spontaneous abortion, premature infant with toxemia, growth-restricted infant?

• a medical patient on bedrest?

• having minor surgery?

• had a history of major surgery in past month?

• have swollen legs?

• have central venous access?

• having arthroscopic surgery?

• having major surgery (any procedure lasting > 45 minutes)?

• have a malignancy (current or previous)?

• having a laparoscopic procedure lasting > 45 minutes?

• a patient confined to bed > 3 days (> 72 hours)?

• had an immobilizing plaster cast during past month?

• have a history of deep vein thrombosis or pulmonary embolism?

• have a thrombophilic condition (Factor V Leiden, prothrombin G20210A, elevated serum homocysteine)?

• have a positive lupus anticoagulant?

• have heparin-induced thrombocytopenia?

• have a family history of venous thromboembolism?

• have elevated cardiolipin antibodies?

• had a stroke in past month?

• had multiple trauma in past month?

• having elective major lower extremity arthroplasty (knee or joint replacement, etc)?

• had a hip, pelvis or leg fracture?

• had acute spinal cord injury with paralysis in past month?
