Has the person lived in or traveled to the Middle East?

Has the patient had contact with animals that may be infected with the MERS coronavirus?

Has the patient had unprotected contact with a patient with MERS coronavirus infection?

Age of the patient

Number of significant comorbid conditions

• fever?

• respiratory symptoms (cough, shortness of breath) and/or signs of pneumonia?

• acute respiratory distress syndrome (ARDS)?

• vomiting and/or diarrhea?

• onset or worsening of renal failure?

Has the coronavirus been positively identified in a sample from the patient?
