• a coronary artery bypass?

• myocardial infarction?

• additional myocardial infarction(s)?

• cardiomyopathy ?

• valvular disease with murmur > 3/6?

• an arrhythmia for > 6 months?

• hypertension requiring therapy for > 6 months?

• claudication lasting > 6 months?

• tissue loss (pulp space, digit, limb, gangrene, amputation)?

• venous thrombosis with swelling, ulceration or venous stasis?
