Description

The Supervision Rating Scale (SRS) can be used to evaluate a patient who has had a head injury. This can help measure the extent to which a person can function either in the community or with supervision after a significant head injury.


 

Supervision indicates that someone is responsible for being with the patient.

 

Parameters:

(1) patient's need for supervision

(2) person providing supervision

(3) environmental controls

(4) extent of full time supervision, if needed

patient unsupervised at night; a person may be responsible to supervise the patient, but this person may be absent sometimes overnight

patient supervised overnight and part-time during the day but may have independent outings without supervision

patient supervised overnight and part-time during the day but is unsupervised during working hours; the supervising person may be absent to work full-time outside of the home

patient supervised overnight and for most waking hours; the supervising person may be gone for more than an hour but not long enough to work full-time

patient supervised overnight and during almost all waking hours; supervising person absent for periods less than an hour

at least 1 person is always present; the person checks on the patient once every 30 minutes or less often

at least 1 person is always present; overnight safety precautions such as outside door being locked
