Infectious causes:
(1) histoplasmosis
(2) aspergillosis
(3) cryptococcosis
(4) blastomycosis
(5) mucormycosis
(6) tuberculosis
Physical or chemical causes:
(1) radiation
(2) methysergide or other drugs
Other causes:
(1) familial
(2) sarcoidosis
(3) IgG4-related sclerosing disease (hyper-IgG4 disease)
(4) other autoimmune disease
(5) idiopathic
