Description

Puskarich et al studied whole blood lactate kinetics in patients with sepsis and septic shock. The authors are from the University of Mississippi, Cooper University Hospital (Camden, NJ), Beth Israel Deaconess Medical Center and Carolinas Medical Center.

 


Patient selection: sepsis and septic shock with hyperlactatemia (>= 2 mmol/L)

 

The initial whole blood lactate was measured at admission.

 

Predictors of survival:

(1) whole blood lactate normalization (< 2 mmol/L) at 6 hours after admission (OR 5.2)

(2) whole blood lactate clearance >= 50% (OR 4.0)

 

where:

• The time frame for second criterion was not clearly stated. Repeat lactate measurements were taken 2-3 hours after the initial. Alternatively, it is at 6 hours to match the first criterion.
