Description

A person with a patent foramen ovale (PFO) and right-to-left shunt is at increased risk for high altitude pulmonary edema (HAPE). PFO is present in up to 25% of the general population, but it may only become clinically evident under conditions when the right atrial pressure exceeds that in the left atrium.

Findings that may indicate the presence of a PFO in a patient with HAPE:

(1) recurrent episodes of high altitude pulmonary edema

(2) onset of HAPE at low altitudes

(3) onset of HAPE despite acclimatization

(4) lower than expected arterial oxygen saturation
