Description

An oral food challenge test (OFCT) can be useful in some clinical situations. It should be avoided in others.

Reasons to consider performing an OFCT:

(1) initial identification of foods causing an allergic response

(2) monitoring resolution of a food allergy

(3) determining if a food associated with a chronic allergic condition like atopy is able to cause an immediate reaction

(4) expanding the diet of a patient with multiple dietary restrictions

(5) determining the tolerance of cross-reacting foods

(6) determining if cooking or other food processing can make a food tolerable to the patient

Reasons to defer performance of an OFCT:

(1) high probability of having a serious reaction to the food

(2) history of anaphylaxis or unstable asthma following exposure to the food

(3) presence of a medication or condition that may interfere with the interpretation of the results

(4) presence of a medication or condition that may make an allergic reaction or its treatment hazardous to the patient
