Description

Infants with thiamine deficiency may develop Wernicke’s encephalopathy. The condition may be misdiagnosed if the clinician is not familiar with it. It can be fatal if the patient is not properly treated.


 

Causes:

(1) breast feeding by thiamine-deficient mothers

(2) thiamine deficient formula

(3) malnutrition

 

Classic features (triad):

(1) altered mental status (lethargy, irritability, excessive crying, confusion)

(2) ocular signs (upbeat nystagmus, ophthalmoplegia, ptosis)

(3) ataxia

 

Other findings:

(1) signs of beri-beri

(2) lactic acidosis

 

MRI of the brain shows involvement of:

(1) frontal lobes

(2) basal ganglia

(3) periaqueductal region

(4) periventricular thalamus

(5) mammillary bodies

(6) braiinstem
