Description

Fulminant necrotizing enterocolitis is a life-threatening condition, and early diagnosis is important for survival. A number of findings may be noted before the clinical deterioration.

Patient selection: premature neonate

Findings that may be noted before the onset of fulminant NEC:

(1) sepsis

(2) abdominal distention

(3) disappearance of bowel sounds

(4) leukopenia with neutropenia and/or lymphopenia

(5) thrombocytopenia

(6) pH <= 7.2

(7) blood product transfusion (red blood cells, platelet concentrate)

(8) serum CRP >= 10 mg/L

The presence of pneumoperitoneum on imaging would be indicative of established necrosis.
