A ratio > 40 is seen in:
(1) prerenal azotemia
(2) acute glomerulonephritis
(3) hepatorenal syndrome
A ratio < 20 is seen in:
(1) acute tubular necrosis (acute oliguric renal failure)
(2) acute obstructive renal failure
(3) acute nonoliguric renal failure
