Description

A positive urine leukocyte esterase result may represent a true or false positive for urinary tract infection (UTI). Certain urine proteins can help to identify a true infection and to identify a patient who does or who does not need antibiotic therapy pending culture.


Patient selection: positive leukocyte esterase test on urine

 

Criteria for a true positive: positive urine culture

 

Urine proteins associated with a true UTI:

(1) IL-2

(2) IL-8

(3) IL-9

(4) NGAL (neutrophil gelatinase-associated lipocalin)

 

The area under the ROC curve was 0.94.
