Description

Both D-arabinitol and L-arabinitol are found normally in serum and urine, while D-arabinitol is a metabolite found in Candida species. An increase in the amount of D-arabinitol relative to L-arabinitol can be found in pediatric oncology patients with invasive candidiasis. The elevation may occur in advance of positive cultures, allowing early and more targeted therapy.


 

Patient Selection:

(1) children with cancer being treated with chemotherapy

(2) fever > 38.3 °C

(3) absolute neutrophil count < 500 per µL

 

Specimen: random urine specimens

 

Testing: gas chromatography

 

D-arabinitol to L-arabinitol ratio =

= (urine D-arabinitol level) / (urine L-arabinitol level)

 

Interpretation:

• The ratio in non-neutropenic children are 2.5 +/- 0.7

• Ratios > 4.6 are considered positive for invasive candidiasis
