Description

An older adult may develop a cytopenia or other hematologic disorder that cannot be explained by the usual suspects. Further evaluation can help to classify a patient into one of several categories.


Features of unexplained hematologic disorder of the elderly:

(1) older adult (typically >= 65 years of age)

(2) hematologic disorder not explained by comorbid condition (malnutrition, renal disease, etc)

(3) workup incomplete

 

Parameters:

(1) cytopenia

(2) clonal or molecular aberration

(3) cytogenetic abnormalities

ICUS = idiopathic cytopenia of unknown significance

CCUS = clonal cytopenia of unknown significance

IDUS = idiopathic dysplasia of unknown significance

CHIP = clonal hematopoiesis of indeterminate potential

MDS = myelodysplastic syndrome

 

The trigger for a workup of ICUS and CCUS would be cytopenia. Recognition for IDUS and CHIP could be delayed.
