Dreischulte and Guthrie listed a number of failures in prescription practices that can lead to preventable hospital admissions. The authors are from the University of Dundee.


Failures in prescription practices that can lead to a preventable hospital admission:

(1) failure to discontinue a drug that is not indicated or no longer indicated

(2) failure to use a drug that is indicated to prevent an adverse drug reaction

(3) failure to take into account existing medical conditions

(4) failure to take into account drug interactions with existing drug therapy

(5) failure to adequately monitor a patient when monitoring is indicated

 

An individual patient may have more than one of these failures.
