Trichomonas species can rarely cause an epididymitis.
Risk factors:
(1) immunocompromised
(2) female sexual partner with Trichomonas vaginitis
Associations may include:
(1) other sexually transmitted infections
(2) disseminated disease
(3) trichomonas urethritis or prostatitis
Clinical findings:
(1) scrotal swelling
(2) tender epididymis
(3) urethral discharge
Laboratory findings:
(1) trichomonads in urine and/or semen
The diagnosis is supported by:
(1) exclusion of other causes
(2) resolution of the epididymitis after metronidazole
