Description

Sciarra et al developed a simple pathological score for evaluating response of a hepatocellular carcinoma to TACE (transarterial chemoembolization). This can help to identify a patient who should or should not get the therapy. The authors are from the University of Milan and Beaujon Hospital in Cichy, France.

Patient selection: hepatocellular carcinoma treated with transarterial chemoembolization (TACE).

Outcome: resistance (failure to respond), with > 50% residual tumor

Parameters:

(1) immunohistochemical staining for VEGF

(2) immunohistochemical vascular staining for CD34

= SUM(points for the 2 immunostains)

Interpretation:

minimum score: 0

maximum score: 3

A score of 2 or 3 was associated with residual tumor > 50% (resistance).

A score of 0 or 1 was associated with residual tumor < 50%.

The result depends only on the VEGF result.

Performance:

The sensitivity was 89% and specificity 59%, with accuracy of 81%.
