TRACH Score of Szeder et al helps to identify a patient with supratentorial spontaneous intracerebral hemorrhage who may get a tracheostomy. The authors are from the Medical College of Wisconsin and Johns Hopkins University.
Patient selection: supratentorial spontaneous intracerebral hemorrhage (sICH) who survived first 3 days
Parameters:
(1) RScale
(2) Glasgow Coma Score
sICH Feature
Points
located in thalamus
2
hydrocephalus
1.5
septum pellucidum shift
3
RScale =
= SUM(points for all of the items)
TRACH Score =
= (RSCALE) - (0.5 * (GCS)) + 3
Interpretation:
• minimal score: -4.5
• maximum score: 4.5 observed in study (max may be 8.0)
• The higher the score the greater the chances of needing a tracheostomy.
• A score < 0.7 was associated with no tracheostomy and successful extubation.
• A score > 2 indicates tracheostomy.
Performance:
• The area under the ROC curve is 0.92.
