Fazel et al refer to use of the tool giving a 9 point scale. There are 2 ways that 9 points could be assigned:

(1) on page 1141, 9 items are listed. These correspond to A2, all B items, and all of the C items, with C3a and C3b counted as one item.

(2) Use all of the B and C items, with C3a and C3b counted separately. I have used this in the implementation.
