Description

Thrombocytopenia may rarely occur following infusion of a radiocontrast material.


Clinical features:

(1) recent infusion of radiocontrast material

(2) sudden drop in platelet count (within a few hours of infusion)

(3) variable presence of petechiae and/or ecchymoses

(4) variable signs of an immediate allergic reaction (skin rash, facial flushing, chest discomfort, etc)

(5) reversible over several days

(6) exclusion of other explanations for thrombocytopenia

 

The mechanism has not been definitively established but most likely is an acute immune reaction.

 

The patient may be treated with corticosteroids or undergo hemodialysis to remove the contrast material.
