Description

The Venous Segmental Disease Score (VSDS) can be used to describe the extent of reflux and obstruction in the venous system of the lower extremities as demonstrated on imaging studies (such as phlebography or color duplex scan). This can be used for initial patient evaluation and for monitoring the venous disease over time.

Component scores:

(1) reflux score (reflux = incompetence in all valves in a venous segment)

(2) obstruction score (obstruction = EITHER total occlusion at some point in the segment OR > 50% narrowing for >= 50% of the segment)

Venous Segment with Reflux

Points

lesser saphenous

0.5

greater saphenous

1

perforators, thigh

0.5

perforators, calf

1

posterior tibial vein alone

1

calf veins, multiple

2

popliteal vein

2

superficial femoral vein

1

profunda femoris vein

1

common femoral vein and above

1

where:

• The posterior tibial vein alone and multiple calf veins are mutually exclusive.

total reflux score =

= SUM(points for reflux)

Obstructed Venous Segment

Points

greater saphenous (if from groin to below the knee)

1

calf veins, multiple

1

popliteal vein

2

superficial femoral vein

1

profunda femoris vein

1

common femoral vein

2

iliac vein

1

inferior vena cava

1

total obstruction score =

= SUM(points for obstruction)

Interpretation:

• minimum score for each: 0

• maximum score for each: 10

• The higher the scores, the more severe the venous disease.
