Description

The International Germ Cell Cancer Collaborative Group have proposed a prognostic system of classification for metastatic germ cell tumors in males, based on the type of germ cell tumor, location of metastases and serum markers.


 

Seminomas

 

Good Prognosis:

(1) any primary site, AND

(2) no non-pulmonary visceral metastases, AND

(3) normal AFP

(4) any hCG

(5) any LDH

 

Intermediate Prognosis:

(1) any primary site, AND

(2) non-pulmonary visceral metastases, AND

(3) normal AFP

(4) any hCG

(5) any LDH

 

Poor Prognosis:

(1) no patients classified as poor prognosis

Non-Seminomatous Germ Cell Tumors

 

Good Prognosis:

(1) testis or retroperitoneal primary, AND

(2) no non-pulmonary visceral metastases, AND

(3) AFP < 1,000 ng/mL, AND

(4) hCG < 5,000 IU/L (1,000 ng/mL), AND

(5) LDH < 1.5 times upper limit of normal

 

Intermediate Prognosis:

(1) testis or retroperitoneal primary, AND

(2) no non-pulmonary visceral metastases, AND

(3) any of the following

(3a) AFP >= 1,000 ng/mL and <= 10,000 ng/mL, OR

(3b) hCG >= 5,000 IU/L (1,000 ng/mL) and <= 50,000 IU/L (10,000 ng/mL) OR

(3c) LDH >= 1.5 and <= 10 times the upper limit of normal

 

Poor Prognosis:

(1) mediastinal primary, OR

(2) non-pulmonary visceral metastases, OR

(3) AFP > 10,000 ng/mL, OR

(4) hCG > 50,000 IU/L (10,000 ng/mL), OR

(5) LDH > 10 times upper limit of normal
