Description

Standardization of the serum creatinine assays initiated by the National Kidney Disease Education Program (NKDEP) has affected equations used to estimated the glomerular filtration rate (eGFR). The Modifiication of Diet in Renal Disease (MDRD) was affected by the change.

Patient population: adults with chronic kidney disease (CKD)

Change: coefficient changed from 186 to 175, with the ratio of 186 to 175 = 1.0629.

Parameters for the equation:

(1) serum creatinine in mg/dL, standardized

(2) age of the patient in years

(3) gender

(4) race

GFR in mL per min per 1.73 square meters BSA =

= 175 * ((serum creatinine in mg/dL)^(-1.154)) * ((age of the patient in years)^(-0.203)) * (gender points) * (racial points)

Performance:

• The equation is useful for GFRs up to 90 mL per min per 1.73 square meter BSA (page S83, Kasiske, 2002).
