Description

Ji et al listed findings that can help to identify cases of nonventilator hospital-acquired pneumonia (NV-HAP). The presence of NV-HAP was associated with greater in-hospital mortality. The authors are from Xi'an Jiaotong University, Harvard Medical School, Brigham and Women's Faulkner Hospital and Massachusetts General Hospital.


Patient selection: patient who has not been on a ventilator

 

Criteria for NV-HAP:

(1) worsening oxygenation

(2) new antibiotic therapy >= 3 days

(3) fever (temperature > 38°C) or abnormal white blood cell count (< 4,000 or > 12,000 per µL)

(4) performance of a chest X-ray

 

Criteria 2, 3 and 4 should occur on the first or second day of worsening oxygenation.

 

Criteria for worsening oxygenation - at least 2 days of stable or improving oxygenation followed by at least 2 days of one of the following:

(1) decrease in daily minimal oxygen saturation from at least 95% in a patient on ambient air to less than 95% on ambient air)

(2) initiation of supplemental oxygen

(3) escalation of supplemental oxygen
