Description

Inhalation of steam can result in severe thermal injury to the upper and lower respiratory tract. Although initial findings may be modest, these injuries can become life-threatening. Patients with a significant exposure need to be monitored closely and managed aggressively.


 

Patient selection: exposure to steam, especially in an enclosed space

 

Early findings:

(1) shock

(2) facial burns, especially around the mouth

(3) sudden onset of hoarseness or stridor

(4) pulmonary atelectasis on chest X-ray

(5) pulmonary edema

(6) pulmonary hypertension

(7) bronchoscopic evidence of injury to oropharynx, larynx, trachea and bronchi

(8) pansinusitis

(9) hemolysis

 

Complications that may develop over the next several days:

(1) hyaline membrane disease and ARDS

(2) respiratory failure

(2) bronchopneumonia

(3) necrotizing tracheobronchitis

(4) desquamative interstitial pneumonia
