Description

Ay et al identified a number of risk factors for venous thromboembolism in cancer patients. These can help to identify a patient who may benefit from closer monitoring and more aggressive management. The authors are from the Medical University of Vienna and the Vienna Cancer and Thrombosis Study (CATS).

Soluble P-selectin is a cell adhesion molecule found in alpha granules of platelets and Weibel-Palade bodies of endothelial cells. Receptors for soluble P-selectin include P-selectin glycoprotein ligand-1 (PSGL-1) and CD24.

Patient selection: cancer, at time of diagnosis

Parameters:

(1) soluble P-selectin

(2) surgery

(3) radiotherapy

where:

• The points are based on hazard ratio from multivariate analylsis, which ranged from 2.6 to 3.9, with p values <= 0.003.

• The upper quartile of the normal range is indicated by the 75 th percentile. This would be approximately the mean plus 0.68 SD.(i.e, "high normal" or above normal).

total number of risk factors =

= SUM(points for all 3 factors)

Interpretation:

• minimum number of risk factors: 0

• maximum number of risk factors: 3

• The risk of venous thromboembolism increases with the number of risk factors.

• The risk of venous thromboembolism with a soluble P-selectin >= 75 th percentile alone was 12% vs 4% if not.
