Description

The SLEDAI-2K score describes the severity and activity of systemic lupus erythematosus (SLE) and was reported in the year 2000 as an update to the original SLEDAI.


SLEDAI = systemic lupus erythematosus disease activity index 2000 (2K)

 

Patient selection: patient with SLE

 

Time frame: initially the time frame was the past 10 days but later could be expanded to 30 days

 

Parameters:

(1) seizure, excluding other causes

(2) psychosis, excluding other causes

(3) organic brain syndrome excluding other causes

(4) visual disturbance, excluding other causes

(5) cranial nerve disorder

(6) lupus headache

(7) cerebrovascular accident (CVA), excluding atheroscerlosis as cause

(8) vasculitis (ulceration, gangrene, tender finger nodules, periungual infarction, splinter hemorrhage, etc)

(9) arthritis involving 2 or more joints

(10) myositis with proximal muscle weakness and aching

(11) urinary casts

(12) hematuria not due to other cause

(13) proteinuria > 0.5 g in 24 hour

(14) pyuria (> 5 WBC per hpf not due to urinary tract infection)

(15) rash

(16) alopecia

(17) mucosal ulcerations (oral or nasal)

(18) pleurisy

(19) pericarditis

(20) low serum complement (CH50, C3, C4)

(21) increased DNA binding (Farr assay)

(22) fever (> 38°C) not due to other cause

(23) thrombocytopenia (platelet count < 100,000 per µL) not due to drug cause

(24) leukopenia (white blood cell count < 3,000 per µL) not due to drug cause

Interpretation:

• minimum score: 0

• maximum score: 105

• The higher the score the more severe and active the SLE.
