Povidone iodine (Betadine) is toxic to bacteria. Under certain circumstances it can be toxic to epidermal cells as well.
Risk factors for a skin burn (skin maceration):
(1) accumulation of liquid povidone-iodone that is not allowed to dry
(2) dependent area or other area subject to abrasion and/or pressure (lithotomy position, tissue under a tourniquet)
(3) other risk factors for decubital ulcer (malnutrition, obesity, etc)
(4) prolonged exposure
Complications:
(1) progression to a decubital ulcer
(2) infection
The risk can be reduced by immediate removal of precipitating conditions.
