Sarcoidosis may rarely affect the paranasal sinuses, nose, larynx and other structures in the head and neck.
Clinical features:
(1) variable history of sarcoidosis elsewhere in the patient
(2) chronic sinusitis and/or rhinitis
(3) variable nasal enlargement
(4) variable osteomyelitis
Primary clinical subgroups:
(1) atrophic
(2) hypertrophic (resembling chronic rhinosinusitis)
(3) destructive
(4) nasal enlargement
The diagnosis is often made by the presence of noncaseating granulomas on biopsy with exclusion of other conditions in the differential diagnosis.
Differential diagnosis:
(1) other forms of chronic sinusitis
(2) other causes of granulomatous inflammation
