Description

Hurabielle et al evaluated patients with systemic sclerosis for skin telangiectasia and correlated them with outcomes. These can help to identify a patient who may benefit from more aggressive management. The authors are from Paris Descartes University, Sorbonne Paris Cite, Hopital Cochin in Paris.

Patient selection: systemic sclerosis

Parameters for skin examination:

(1) number of skin telangiectasia on hands and face (use of "and" given in Table 1)

(2) diameter of telangiectasia in mm on hands and face

Designations based on the examination:

(1) profuse skin telangiectasia: >= 11 skin telangiectasia

(2) pseudotumoral: diameter > 5 mm

Designation

Significance

profuse skin telangiectasia

capillary loss and severe neoangiogenesis;

late nailfold videocapillaroscopy pattern;

past and current digital ulcers;

possibly cardiac involvement

pseudotumoral

capillary loss and severe neoangiogenesis;

late nailfold videocapillaroscopy pattern;

precapillary pulmonary hypertension

where:

• The late nailfold videocapillary pattern is associated with a more severe vascular prognosis.
