Description

A patient with advanced diabetes may develop foot ulcerations. Lormeau et al identified patients with diabetic foot ulcers associated with what they termed the short Achilles tendon syndrome. The authors are from Hopital Jean Verdier in Bondy, France.

Features of the short Achilles tendon syndrome:

(1) limited dorsiflexion of the tibiotarsal joint

(2) correction of limited mobility by tenotomy of the Achilles tendon

A patient with severe diabetic neuropathy may develop a talipes equinovarus deformity with increased pressure on the forefoot (see Chapter 21). This increase in pressure may predipose the foot to diabetic ulcers.

The authors reported patients with diabetic foot ulcerations who showed significant improvement following the Achilles tendon tenotomy.
