Description

Hreinsson et al reported the SHA2PE score for patients with lower gastrointestinal bleeding. This can identify a patient who is unlikely to receive a hospital-based intervention. The authors are from Landspitali and University of Iceland in Reykjavik.

SHA2PE is an acronym for the score's parameters.

Patient selection: lower GI bleeding

Hospital-based intervention: transfusion, endoscopic hemostatic therapy, arterial embolization, surgery

SHA2PE is an acronym for the score's parameters.

Parameters:

(1) systolic blood pressure in mm Hg

(2) hemoglobin in g/dL

(3) antiplatelet therapy

(4) anticoagulant therapy

(5) pulse in beats per minute

(6) emergency room bleeding

Interpretation:

minimum score: 0

maximum score: 7

A patient with a score <= 1 was highly unlikely to have a hospital-based intervention.

Performance:

The area under the ROC curve was 0.83.

The negative predictive value was 96%.
