Description

Intracavernosal injection of vasoactive substances can help certain patients with erectile dysfunction. Patients with certain medical conditions should not be tested by intracavernosal injection of vasoactive substances. Additional criteria should be used to identify those patients likely to be capable of self-injection and those likely to have significant complications.


 

Vasoactive substances (papaverine with phentolamine and/or PGE1) may be injected alone, or combined with other therapies such as use of a vacuum device.

 

Contraindications for testing by intracavernosal vasoactive injection:

(1) severe decompensated cardiovascular insufficiency

(2) critical coronary artery disease

(3) severe disturbances of liver function (papaverine)

(4) glaucoma (papaverine)

(5) severe benign prostatic hypertrophy with increased residual urine (papaverine)

 

Criteria for selection of patients for autoinjection with vasoactive agents:

(1) cooperative and reliable

(2) adequate followup

(3) trained in handling possible side effects

 

Contraindications for autoinjection therapy with vasoactive agents:

(1) decompensated cardiac and circulatory insufficiency

(2) intellectual incompetence

(3) lack of compliance

(4) severe psychiatric disorder

(5) sexual deviation
