The Pirogoff procedure for hindfoot amputation can provide a good outcome in selected patients. Nather listed criteria to select patients for this procedure.
Selection criteria - all of the following:
(1) palpable posterior tibial artery pulse
(2) ankle-to-brachial index (ABI) > 0.7
(3) hemoglobin > 10 g/dL
(4) serum albumin > 30 g/L
The presence of poor perfusion is associated with worse outcomes.
Advantages of a Pirogoff procedure:
(1) weight-bearing stump which is capable of load-bearing
(2) ability to ambulate a short distance without wearing a prosthesis
(3) small limb length discrepancy
