where:
• The age limits are from the 1993 ATS recommendations. Earlier reports used younger cutoffs (< 35 years for heart-lung and < 55 years for lung only).
• Griffith and Zenati use PaO2 > 300 mm Hg on 100% oxygen as the cutoff.
