Description

Holland et al reported a screening tool for specialized discharge planning services. This is a precursor to the Early Screen for Discharge Planning (ESDP). The authors are from the Mayo Clinic.

Interpretation:

minimum score: 0

maximum score: 23

The higher the score the greater the need for discharge planning.

A cutoff > 10 has a sensitivity of 79% and specificity of 76%.

A score > 11 has a sensitivity of 75% and specificity of 78%.

 

Performance:

The area under the ROC curve is 0.84 in the development cohort and 0.822 in the validation cohort.
