Screening Questionnaire of Blum to Identify Previous Injury or Medical Conditions Prior to Sports or Exercise Participation

The instrument consists of 11 questions related to previous injuries or health that may impact sports participation.

• The questionnaire asks for the date of the last tetanus toxoid and polio immunizations. I added a question to the implementation asking if DPT vaccinations are current.

Any positive responses to the above screening questions should be investigated before clearing the person for participation.
