Barreto et al used the sarcopenia index (SI) to screen critically ill patients for malnutrition. Malnutrition is common in critically ill patients and is associated with a worse prognosis. The authors are from the Mayo Clinic, UPMC Pinnacle, and Tufts Medical Center.
Patient selection: critically ill
Parameters:
(1) serum creatinine in mg/dL
(2) serum cystatin in mg/L
sarcopenia index =
= (serum creatinine) / (serum cystatin) * 100
Interpretation:
• The SI is lower in malnourished patients.
• Malnourished patients had a mean SI of 64 (+/- 27) vs 72 for well-nourished patients (+/- 25).
• An SI <= 43 has a hazard ratio of 2.6 for mortality.
Performance:
• The area under the ROC curve is 0.61.
