Additional information to collect:
(1) family history of high cholesterol
(2) premature coronary artery disease (< 55 years in males, < 65 years in females)
(3) tendon xanthomas
(4) arcus corneae at age < 45 years
(5) tuberous xanthomas or xanthelasma at age < 20 years
