(1) birth weight in grams or gestational age in weeks
(2) clinical course

<= 1500 g OR <= 30 weeks

1501 to 2000 g OR > 30 weeks
unstable clinical course (cardiorespiratory arrest, other)
Retinal screening exam is performed after pupillary dilation by using binocular indirect ophthalmoscopy with a lid speculum and scleral depression.
