Description

Huang and Lin listed screening criteria for weaning of a patient on mechanical ventilation. The authors are from Chang Gung Memorial Hospital in Taipei, Taiwan.


 

Physiologic screening criteria - all of the following:

(1) heart rate < 140 beats per minute

(2) oxygen saturation > 92%

(3) FIO2 < 35%

(4) cough reflex intact

 

Exclusion of high-risk conditions:

(5) absence of unstable coronary artery disease

(6) absence of increased intracranial pressure

(7) absence of other unstable features (FIO2 > 50%, vasopressors required to maintain blood pressure, temperature > 39°C or < 36°C, heart rate > 150 beats per minute, respiratory rate > 35 or < 8)

 

Ventilator-associated factors:

(8) PEEP < 8 cm H2O

(9) pressure-support mode ventilation with pressure < 8 cm H2O

 

Since the paper demonstrated that adrenal insufficiency was associated with impaired weaning, an additional criteria can be added:

(10) adequate adrenal reserve or appropriate hydrocortisone supplementation
