Description

Nair et al reported the Irish Maternal Early Warning Score (IMEWS). This can help to identify a pregnant woman who may be septic. The authors are from St. Vincent's University Hospital. National Maternity Hospital Dublin, and Mater Misericordiae University Hospital in the Republic of Ireland.


Criteria for maternal sepsis - both of the following:

(1) >= 2 SIRS criteria present

(2) infection suspected after medical evaluation

 

SIRS criteria:

(1) temperature < 36.0°C or >= 38.0°C

(2) respiratory rate >= 20 breaths per minute

(3) heart rate >= 100 beats per minute

(4) WBC count < 4,000 per µL or > 16,900 per µL

(5) blood glucose > 7.7 mmol/L in the absence of diabetes

(6) acute change in mental status

 

number of SIRS criteria present =

= SUM(points for all of the parameters)

 

Clinical intervention to suspected maternal sepsis:

(1) complete "sepsis six" (3 takes plus 3 gives) within 1 hour

(2) "three takes": appropriate cultures; urine output every hour; CBC and serum lactate

(3) "three gives": oxygen saturation maintained >= 94%; consider IV fluid bolus; IV antibiotics
